You know what? The reality is: I know where to get (quality) website design templates for one dollar. I know how to obtain logos -which look as good as *any* tech company logo- for one or two dollars. I’ve been doing this for 14 years, and I’ve learned all the tricks. But I don’t go flaunting this in here, or undercutting other designers. I want people in this subreddit to thrive. It’s quite understandable that people would get irked when someone asks for quality work for chump rates. I looked at that restaurant site example, and I knew right away that it wasn’t worth my time for $200. Designing something like that from scratch takes real time. It’s a good deal of work. And then of course the client will want changes, and realistically you’re looking at being busy for a couple of days, if not more. I’m not sure there’s any easy answers to this. I myself am a programmer as well as a designer. I’m phasing my design skills out, for the reasons I outlined above. If you want to allow this subreddit to devolve into a “lowest common denominator” situation, well then keep doing what you’re doing, because that’s what’s going to happen. You are just starting out so let me give you the most important advice. DO NOT charge $50 for a website. Maybe every now and then for a sale or something for some simple site. But do not make it your standard price. Web designers command $300 and up for small websites. Your hourly if you go that route should be between $30 and $50 per hour.
You should let the customer register their domain. Or if they have no clue about anything web related then offer to do it for them. You will also need Paypal for your payments. Another good merchant is Alert Pay which is similar to Paypal. Either way you need to verify the account in order to be legitimate and be able to get the money into your hands.

Some thoughts on your questions:
If you get an internship, make sure it is payed, or extremely prestigious. Ask a lot of questions while you are there, remember you are there to learn.
If you've got your own clients, just let your employer know you have good experience working with customers. Otherwise, keep your own clients to yourself.
To answer question 3: I studied Photography for two years then worked in the "real world" for a while, I taught myself a lot about graphic design over the years, and am now working towards an architecture degree. I use every scrap of info I've learned in each discipline. So yes, you should have a strong background in design too, particularly user interfaces, typography and working with grids. However, you should also pursue any other interests you may have. They wont be a waste, they will help give your work more personality and a stronger aesthetic.
also, you should create a web and printed portfolio. The portfolio should focus on your graphic and artistic work, the web should show your understanding of building logical systems, and solving problems: it shouldn't look to fancy.
One caveat about using templates- these are as generic and blank as they get, and to sell your services your website needs to be the least generic and blank design possible. That means you're required to put at least some work into its design, so you really should have some idea on what your web site layout should look like before continuing. The design probably will change in mid development as you come up with new ideas but at least you'll have a direction in which to proceed.
That said, I hate to sound like a commercial, but if you want to start a small business website with minimal fuss then Yahoo's small business services (smallbusiness.yahoo.com) are superb. Domain, web site, and email is $110 per year. Included in the service is a generic library of some 80 blank templates which you can access and modify immediately, but if you have some website design skills you can download their free utility called SiteBuilder. It's a complete web development system and has another 144 templates. The great thing about SiteBuilder is that once you finish your design, simply click the PUBLISH botton on the toolbar and it does all the work for you to automatically upload your design to your domain so it can be viewed by the public immediately.
If you're crunched for cash or you already have your domain and don't need email, then NVU is a good editor (www.nvu.com). It's an open source web development application and it's amazingly powerful for a free program. There are gigantic libraries of free templates created by artists who want to share their work, such as that you can download and modify as you wish. I just finished creating a website for a local town political party running on a shoestring budget, and the above tips worked wonders.
This is a tall order mate, seeing the sample website http://www.reed.co.uk it's a site with data base and that costs a lot of money… probably around US$3k min. but if you just need the design that would be cheaper, may less than 500 for the lay out…
You can either hire freelancers which is cheaper or go to a more stable web development company more expensive but it's a one stop shop.
